Home Care Coordinator (Pool)

Overview : Pool position; will be working weekday hours (Monday - Friday) at several Banner hospitals as staffing needs arise; no weekends.

Responsibilities :

The Home Care Coordinator- RN position is designed to facilitate the seamless coordination of patient care by successfully expediting the intake of community-based referrals. This per diem pool position is integral tof the Banner Health System in order to integrate services wit acute and long-term services. This person will be required to work every other weekend, and may float to designated banner hospitals during the week to facilitate home health referrals. The home care coordinator RN/LPN organizes, gathers, and records information that is necessary to initiate the plan of care for all referrals (i.e Home Health, Home Infusion, Home Medical Equipment, and Hospice) in accordance with State and Federal Regulations, JCAHO Standards, and per policy for program admission criteria.

Qualifications :

RN with current licensure in the State of Arizona.
Minimum of five (5)years of health care experience, two of which are home care related. One year sales/marketing experience preferred.
Current Arizona Drivers License
Current proof of insurance
Excellent customer service, communication, time management, and problem solving skills, and ability ro remain calm under pressure.
