Clinical Care Manager - RN, LPN, LSW

We’re looking for an experienced RN who is self-motivated and adapts well to change. The person in this field-based role is responsible for evaluating and monitoring long term care services for frail and elderly members in their community or home-based facilities. Primary responsibilities include:
Identifying health risk factors in patients
Educating members on care and services
Providing care coordination
Managing Medicare and Medicaid insurance benefits
Serving as a liaison between members and primary care providers
Speaking with physicians for updates on patients



This role requires an LPN, RN or a licensed social worker. Solid long term care or case management experience is also required. MS Office proficiency is a must. Knowledge of Medicare or Medicaid benefits is preferred. Candidates should possess strong organizational and time management skills.
