Essential Duties and Responsibilities
Responsible for developing, documenting, implementing, and evaluating, for each caseload consumer, the nursing care plans and nursing care as required by the Individual Support Plan (ISP).
Attend consumer specialist appointments and follows-up on the outcome of all completed consumer medical appointments.
Monitor vital signs, weight charts, immunization records and health status of consumers.
Review each consumer?s Medication Administration Record and Treatment Sheet on a weekly basis to assure that errors are addressed promptly and/or prevented from occurring.
Visit with each consumer to assess and evaluate medical/nursing care program and to review all medical and health related records.
Work cooperatively with OM?s and QMRP?s to translate identified consumer medical needs into direct-care staffing training requirements.
Communicate at regular intervals, as specified by the ISP, with the consumer or appropriate person regarding the consumer's medical condition, developmental and behavioral status, risks of treatment, and consumer's right to refuse treatment.
Assess the proficiency of direct-care staff in passing medications and completes audits of staff med-pass capabilities.
Participate in the pre-admission evaluation of proposed consumers, case reviews, and development of nursing care and discharge plans.
Assist in staff training, curriculum, County medication training, CPR, and first-aid in the presence of an accident or illness.
Administer and read TB skin tests as necessary for consumers and staff.
Complete medication audits, monthly nursing summary, and quarterly nursing summary.
Maintain consumer medical records in a manner consistent with HIPAA regulations, other Federal and State regulations and applicable DSA, Inc. policies and procedures.
Other duties as assigned.
Qualifications Degree from an accredited school of nursing.
Current state licensure as a licensed practical nurse.
One year of clinical practice preferred.
One year of experience working with individuals with developmental disabilities or in a related field preferred.
Maintains credentials necessary to teach Heartsaver CPR and Heartsaver First Aid courses.
Maintains certification necessary to administer and read tuberculosis skin tests.
Must be at least eighteen (18) years of age.
Ability to communicate effectively; both orally and in writing.
Ability to use common word-processing, spreadsheet and communications software programs within a Windows environment.
Satisfactory completion of a criminal background check as required by federal and/or state laws and any other post-offer pre-employment screenings as mandated by a governing agency or DSA, Inc.
Must possess a valid Indiana driver?s license, a good driving record and a current automobile liability insurance policy
