LVN / Health Care Coordinator

Alterra Healthcare Corporation, a Brookdale Senior Living company, currently seeks an experienced LVN or RN / Health Care Coordinator for the Sterling House of Littleton, an established provider of assisted living services. This is an exciting opportunity for the right healthcare professional.

 

As a LVN or RN/HCC you will deliver and oversee the day-to-day clinical operations of our community. Responsibilities include: managing the health, functional and psycho-social status of residents, initiate individualized service plans and on-going assessments, perform on-site assessments of residents admitted, and conduct preadmission screening of prospective new residents. You will provide training, supervision and monitor staff in the administration of medications, the maintenance of resident charts, review documentation performed by care giving staff, and meet with family members on a regular basis regarding their loved ones care.

This position requires 24/7 call and may share manager on duty responsibilities.

Qualified candidates will possess an LVN or RN state nursing license and a minimum of 2 years experience working with elderly and memory impaired with working knowledge of basic computer access is required.  Supervisory experience is preferred.
