We are seeking exceptional candidates for a TEAM NURSE (LVN or LPT) for our Telecare, Los Angeles Service Area 4 program. Telecare, L.A. 4 is composed of two voluntary programs: an Assertive Community Treatment Program (ACT) and a Homeless Outreach Program (HOP). Both have the purpose of supporting members in the recovery of life roles and hope for a meaningful life. They use objective clinical information to work with members to develop choice-making skills that reduce harm, including clinical risks and increase personal strengths. Supports are provided when skills are not yet developed.

 

Responsibilities: The Team Nurse provides or ensures the provision of services needed or wanted in the most appropriate environment on a regular and emergency basis.

 

Requirements: Licensure as an LVN or LPT by the State of California is required, as is graduation from an accredited school of nursing. Psychiatric experience preferred.  A valid CPR Certificate is also required.  One year experience in direct provision of psychiatric nursing services, one year experience in psychiatry, preferably with chronic mentally ill.  Necessary skills include the ability to work with members with multiple disabilities. Applicant must be finger printed, sign under penalty of perjury that there has been no conviction of any crime, other than minor traffic offenses, and receive clearance from the D.O.J.
