Primary Care Physician
JOB TITLE: PRIMARY CARE PHYSICIAN
SALUD CLINIC
SCHEDULE: Attend clinic 22 hours per week, including 2 hours/week administration time; on-call rotation for emergency and after hours including hospital admissions. Clinics include sites in West Sacramento and Woodland. Additional hours available as needed. Some travel to meetings required.
OVERVIEW: The Primary Care Physician works with a team of health care staff to provide outpatient and inpatient care for clients. The physician works as a part of a multidisciplinary team composed of the clinic manager, practitioners, medical assistants and health workers to provide comprehensive care. The clients are primarily low income, non-English speaking, Medi-Cal recipients or uninsured. The CommuniCare Health Centers? mission emphasizes supportive, nonjudgmental care, preventive care, health education and community referrals to address the broad spectrum of health and psycho-social concerns of clients served.
RESPONSIBILITIES:
The following reflects management?s definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.
1. Serve as one of two Primary Care Physicians to patients over varied time periods. Patients (aged 2 days to 90+ years) may present a wide variety of health problems requiring diagnosis and the selection and management of appropriate care. Such care would include, as necessary, admission to institutional care, referral to specialists, and the coordination of these and other types of care through diverse resources.
2. Coordinate and direct appropriate care by means of initial diagnosis and treatment, obtaining second opinions as necessary, consultation with referral specialists and follow-up of care to assess the results of the primary care, medication regimen and special treatment within the framework of integrated continuous care.
3. Record completely and legibly all patient visits, treatment, communication with patients, treatment, and consultation reports in the medical record.
4. Participate in the CommuniCare Health Centers? utilization review policies and procedures.
5. Participate in the quality assurance program.
6. Supervise nurse practitioner and physician assistant staff assigned to the clinic. Assist with training, supervising and evaluating clinic staff and volunteers.
7. Perform administrative tasks required by clinic licensure, hospital privileges, and funding agencies.
8. Assist in the general oversight of clinic operations to ensure effective health care, efficient working conditions, and client-focused services.
9. Attend other meetings and perform other duties as needed.
MINIMUM POSITION REQUIREMENTS:
Education: Graduation from an accredited school of medicine, current California licensed, Board certified or Board eligible in Family Practice or Internal Medicine. Successful completion of a three-year accredited residency program or active hospital-related practice for at least seven years.
Experience: Experience working in a health clinic or demonstrated commitment to low income populations. Demonstrated commitment to controlling outpatient and inpatient specialty and procedure costs.
Special Skills/Equipment: Bilingual Spanish/Russian/Urdu/Punjabi preferred. Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients serviced in the clinic practice including the principles of growth and development over the life span and have the ability to assess data reflective of the patient?s requirements relative to his or her age specific needs. Ability to operate all clinical equipment in use in the clinic site. Effective communication skills and ability to encourage patient understanding of prevention practices and early diagnostic services. Must provide proof of current driver?s license and auto insurance.

Required to have versatility, flexibility and a willingness to work within constantly changing priorities with enthusiasm and have acute attention to detail. Expected to have strong interpersonal skills and have the ability to understand and follow written/verbal instructions. Able to work independently with little or no supervision. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.
Physical Requirements: Ability to see colors, ability to bend and move arms with sufficient range of motion to complete patient related requirements, hearing required for patient assessments, may need to assist patients on to exam table. Prolonged standing and occasional sitting required. Ability to bend, kneel, lift and twist as needed.
