Pharmacist Infusion (Pool, East Valley)

Responsibilities : The pharmacist is responsible for the delivery of pharmaceutical care, education, and the comprehensive assessment, with integrated team members, to the patient. Responsibilities include overseeing and checking the activities of the pharmacy technician, ongoing evaluation and documentation of all components of pharmaceutical care delivered to the patient, performing clinical monitoring, documenting patient care planning, and communication recommendations and interventions to the health care team. The pharmacist must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. Our patient age ranges from newborn to age 110

Qualifications :

1. Bs degree of pharmacy or Doctor of Pharmacy degree.
2. Must be computer literate
3. Must be knowledgeable of the variety and complexity of pharmaceuticals, the types of medication orders, and be able to fill medication orders accurately and in a timely manor.
4. Must be a licensed pharmacist in the state of Arizona.
5. Must possess both verbal and written communication skills, interpersonal relationship skills and problem solving skills.
6. Proficient in clinical patient monitoring, aseptic technique, and intravenous solution compounding.
7. Preference of 2-3 years experience in a health care system.
