Case Management, Supervisor (RN)
Our Client is among one of the most experienced managed healthcare companies. They specialize in serving patients that are covered under Medicaid, the Healthy Families Programs, the State Children's Health Insurance Program, and other government-sponsored health insurance programs. They have locations in several other states and have been in business for quite some time.
Position Summary
Under the supervision of the Manager, Health Care Services, coordinates the daily operations of Medical, CCS, and Perinatal Case Management services. Supervises professional, technical and support staff. Assures the delivery of comprehensive, coordinated care services over a continuum of time and care settings, in accordance with our client's policies, procedures, and processes. Interacts with the Medical Director, department staff, and providers to ensure timely facilitation of Case Management services.
Functions and Duties
-Functions as direct supervisor of staff in all Case Management (CM) areas, responsible for daily administrative functions, including staffing, productivity, and work quality.
-Assists in interviewing potential CM staff. Orients, trains, and evaluates the work performance of CM staff. Consults with HCS Manager to devise and implement corrective action or disciplinary measures necessary to improve staff performance.
-Acts as both a clinical and administrative resource to CM staff in assessing member needs, devising/implementing plans of care, facilitating CM process, and communicating/coordinating CM activities.
-Collaborates with and keeps the HCS Manager apprised of operational issues, staffing issues, system and program needs.
-Assists with development of department policies and procedures, all CM programs, and development and evaluation of periodic work plans.
-Attends/participates in departmental, company-wide, and external committees, task forces, or work groups, as assigned.
-As liaison to both internal and external customers on behalf of both our client and the CM areas, conducts self in professional manner at all times. Maintains cooperative and effective workplace relationships and adheres to company Code of Conduct.
-Monitors schedules and deadlines for required reports. Assures oversight and direction of individuals primarily responsible, and assures timely completion.
Education Requirements:
Minimum: Bachelors degree in Nursing or related field
Preferred: Masters degree in Nursing or related field
Experience Requirements:
Minimum 5 years clinical nursing experience, with 3 years in Case Management.
