Primary responsibilities include:



Managing complex cases and coordinating care across additional Ovations sites


Identifying high-risk patients and assessing needs


Coordinating care with community partners 



The chosen candidate must be an RN with a current, unrestricted license in the state of Arizona. Significant clinical experience is needed, and prior case management experience is a must. Knowledge of Medicare issues is highly preferred. Spanish (bilingual) skills are a plus. Excellent communication and leadership skills will be valuable.
