Peachtree Hospice-Texarkana is looking for an energetic individual to fill our full-time Director of Clinical Services (Director of Nursing) position with our patient care team.  This position is located in Texarkana, AR with responsibilities over staff and patients in both Texas and Arkansas.  We have seen staff and census numbers more than double in the last 6 months and we need and individual to grow along with us.Our office is part of a network of hospice providers that has 5 offices in 3 states regionally. 

 

 

Description:

The DOCS is directly responsible for the day-to-day direction of our growing staff of nurses, social workers, chaplains, and CNA's.  This individual provides coordinated care to (diagnosis-specific) patients by developing, monitoring, and evaluating interdisciplinary care.  This position requires RN licensure, preference given to applicants with BSN.  Must have a minimum of 3 years' of nursing experience, preferably in hospice or home health, with a minimum of 1 year management experience.

 

Duties:Orients and educates patients and their families by meeting them; explaining the role of the patient care coordinator; initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements.

 Develops interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers; resolving issues that could affect smooth care progression; fostering peer support; providing education to others regarding the case management process.

 Supervises patient care team in the delivery of patient/family care.  Supervises and assesses the quality of services in relation to patient care.

 Monitors delivery of care by documenting care; identifying progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; reviewing the care plan with patients in conjunction with the direct care providers; interacting with involved departments to negotiate and expedite scheduling and completion of tests, procedures, and consults; reporting personnel and performance issues to the Executive Director; maintaining ongoing communication with utilization review staff regarding variances from the care plan or transfer/discharge plan.

 Must be available to work a flexible schedule depending on case load and census demands.  This individual may be asked to work administrative call or make home visits.

 Evaluates outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team; implementing team recommendations.

 Respects patients by recognizing their rights; maintaining confidentiality.

 Maintains quality service by establishing and enforcing organization standards.

 Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; and participating in professional societies.

 Contributes to team effort by accomplishing related results as needed.
