Hospice Intake RN (Weekend pool)

Overview : Pool position to cover weekend intake within Banner Hospitals for hospice.

Responsibilities :

The hospice coordinator is designed to facilitate the seamlesss coordination of patient care by successfully expediting the intake of community-based referrals. This position functions as a member of the hospital and commuity interdisciplinary team and is responsible for assessing and coordinating appropriate home and community services for clients of Banner Hospice. This position is integral to the total care philosophy of Banner Heatlh in order to integrate community-based services with acute long-term care services. This position will interface with internal and external customers of Banner Heatlh and the communities that is services. The hospice coordinator organizes, gathers, and records information that is necessary to initiate the plan of care for all referrals. (ie Home Medical Equipment and Hospice) in accordance with State and Federal Regulations, JCAHO Standards, and per

Qualifications :

Nursing background required. RN required with current nursing licensure in the State of Arizona. Must have three (3) years health care experience, two of which are home care related. One year marketing experience preferred. Current Arizona Drivers License, Current proof of insurance. Excellent customer service, communication, time management, and problem solving skills, and ability to remain calm multiple task situations.
