Exciting Opportunities

1) Case Manager

Assists in the collection of assessment information and service planning and delivery, by locating, referring for, providing and monitoring services to consumers in accordance with their individualized service plan (ISP). Assist with ISP and Complete 6 Month Reviews. Communicates and documents consumers progress toward their recovery.

Requirements - BA degree, no mental health experience, AA degree, 2 years mental health experience, GED and 5 years mental health experience

Valid drivers license and vehicle required for use providing home visits.

 

1) Clinical Liaison

Provides clinical leadership and oversight of service delivery. Responsible and accountable for ensuring that consumers' behavioral health needs are met. Works with the treatment team, including the consumer, their family, community providers and others to identify and implement individualized care and services. Provides coaching and mentoring to the assigned case manager.

Requirements - BA degree in Social Work or Psychology and 4 years mental health experience.
