Medical Social Worker (BSW)

Overview : Banner Lassen Medical Center in Susanville, California is situated at the foot of the scenic Sierra Nevada Mountains. This 25-bed acute care facility is located in the northeast corner of California near the Nevada and Oregon borders. Responsibilities :

Upon request by Care Coordination Deoartment, assesses the client's psycho-social needs and formulates a plan of treatment consistent with diagnosed needs in coordination with and as ordered by the physician. Provides counseling services to clients and families in areas such as interpersonal relationships, stressful life events, disabling diseases, chemical abuse, terminal illnesses, and adjustment to treatment. Performs individual and family counseling which is specific to the client and their family according to age specific needs. Assists and advises clients and families on: insurance benefits, obtaining appropriate financial resources, applying for financial assistance, finding emergency housing, and arranging legal matters. Serves as a client/family advocate. Participates in the discharge planning process and community referral, which includes: making transportation arrangments, arranging for LTC or extended care facility placement and assisting in finding housing and employment. Documents all social services provided, thoroughly, timely and accurately. Plans of care, documentation and care provided must comply with applicable Medicare, State, Federal, JCAHO, insurance and payor requirements as well as local facility policy.

Qualifications :

Masters Degree from a school of social work accredited by the Council of Social Work education required. Two (2) years, minimum, experience in a health care setting preferred. Experience working with complex psycho-social problems, working with individuals and groups, applying a wide range of counseling and intervention techniques and applying social work principles in a medical setting is required.
